Urgent Bite 321 - Hand Foot and Mouth Disease and considering the Rule Bias
2026/08/28
Hand foot and mouth disease is a common viral illness that almost always affects children under 5.   But, as the world's number one golfer shows, healthy adults can contract it, so do not discount it as a possible diagnosis, if the story fits.     Check out the Dermnet NZ page by Leong and Mahon   Read about Scotty Scheffler's HFMD experience.   Check out the paper mentioned Blanco-Alonso S, Mellor-Pita S, Alfageme F, Najera Botella L, Duran-Campo P, Tutor-Ureta P. Urgent Bite 320 - Smith's Fracture and reflections on opinions
2026/08/21
Smith's fracture of the wrist is the opposite of a Colles fracture and is an important fracture to identify in urgent care.   Smith was a successor to Colles, and disagreed with Colles' definition of the Colles fracture.  This offers an opportunity to reflect on the way we use opinions in medical practice, how opinions may vary and must be open to change.     Check out the Radiopaedia page Gaillard F, Stanislavsky A, Smith fracture. Reference article, Radiopaedia.org    If you would like to read Smith's original treatise on fractures, you can find it at this link.   Read from page 130 for the discussions around Colles, and from page 160 for the description of Smith's fracture   www.rnzcuc.org.nz [email protected] https://www.facebook.com/rnzcuc https://twitter.com/rnzcuc   Music licensed from www.premiumbeat.com Full Grip by Score Squad   This podcast is intended to assist in ongoing medical education and peer discussion for qualified health professionals.  Please ensure you work within your scope of practice at all times.  For personal medical advice, always consult your usual doctor
Urgent Bite 319 - Opioid Side Effects
2026/08/14
We commonly see opioid side effects.  But there are some we may not have been aware of that we should keep in mind in patients who have been taking opioids for more than a month.     Check out the Medsafe Prescriber Update - June 2026   www.rnzcuc.org.nz [email protected] https://www.facebook.com/rnzcuc https://twitter.com/rnzcuc   Music licensed from www.premiumbeat.com Full Grip by Score Squad   This podcast is intended to assist in ongoing medical education and peer discussion for qualified health professionals.  Please ensure you work within your scope of practice at all times.  For personal medical advice, always consult your usual doctor
Urgent Bite 318 - Topical Steroids via Urgent Care
2026/08/07
Topical steroids are commonly prescribed through urgent care, but we must be mindful of adding to inappropriate use and inadvertent misuse by patients.   Patients must be clear about how much to apply and how often it must be used.   They need to be aware of the potency of each preparation.  Changes to labelling in 2028 in NZ will help to show how strong each cream is.     Check out the Medsafe Prescriber Update - June 2026 Check out the DermNet NZ Page on Topical Steroids by Amanda Oakley   www.rnzcuc.org.nz [email protected] https://www.facebook.com/rnzcuc https://twitter.com/rnzcuc   Music licensed from www.premiumbeat.com Full Grip by Score Squad   This podcast is intended to assist in ongoing medical education and peer discussion for qualified health professionals.  Please ensure you work within your scope of practice at all times.  For personal medical advice, always consult your usual doctor
Urgent Bite 317 - Splenomegaly
2026/07/31
The spleen, once thought to be the seat of anger, can indicate many different underlying diagnoses and palpating it in urgent care is important.   Check out the paper mentioned Aldulaimi S, Mendez AM. Splenomegaly: Diagnosis and Management in Adults. Am Fam Physician. 2021 Sep 1;104(3):271-276. PMID: 34523897. Urgent Bite 316 - Sinusitis antibiotic choice
2026/07/24
A recent large study in JAMA showed amoxicillin is as effective as augmentin for treating sinusitis.  Which tallies with our national antibiotic guideline advice.  It is important to recognise, sinusitis does not usually require antibiotic therapy.   Check out the paper mentioned Savage TJ, Butler AM, Kronman MP, Durkin MJ, Sreedhara SK, Kabbani S, Hicks LA, Huybrechts KF. Amoxicillin-Clavulanate vs Amoxicillin for Acute Sinusitis in Adults. JAMA. 2026 May 19;335(19):1694-1705. doi: 10.1001/jama.2025.26902. PMID: 41999289; PMCID: PMC13092118. Ehlers-Danlos syndrome - a commonly misunderstood group of conditions. Clin Med (Lond). 2014 Aug;14(4):432-6. doi: 10.7861/clinmedicine.14-4-432.
