How I Actually Run My PT Business (Systems + Workflow)

Most content about running a physical therapy practice is built for multi-provider clinics with full admin teams. I work a roughly 25-hour cap on my week. Half of that is client-facing. The other half is the business. In this episode I break down the three systems that go into those hours — sales, marketing, and client delivery — and what each one actually looks like day to day for a solo practice owner.

How Much Does It Cost to Rent a PT Practice Space?

The listing said twenty-five dollars a square foot. Six hundred square feet. You did the math — fifteen thousand dollars, maybe doable in a year.
Then somebody had to tell you commercial rent is quoted per square foot per year. That's twelve hundred and fifty dollars a month. Before anything else.
In this episode I break down what renting space for a cash based physical therapy practice actually costs once you add in everything nobody warns you about — deposits, triple net charges, buildout, insurance, utilities — and more importantly, how to know if you're actually ready to sign anything.
We cover why the benchmark you'll find online (rent at 5-12% of revenue) is real but the examples aren't built for you, the revenue you need to be generating before a space makes financial sense, the on-ramp most practice owners skip entirely (gym rental, hour-by-hour, sublease), why you are not a retail store and should stop paying for foot traffic your business doesn't need, and the three-question test to know whether a space actually grows your business or just your expenses.
My own numbers: I hit $10,549.17 in month thirteen. I did not have my own space. I still don't. The space was never the thing.

When to Invest in Business Coaching as a Physical Therapist

The month I invested ten thousand dollars in coaching, I made five hundred and four dollars and seventy cents.
Most people hear that and think — that's terrifying. But here's what I know now that I didn't know then: waiting until you're more financially stable to invest in coaching is one of the most expensive habits in business. And jumping in before you're ready wastes money too.
So how do you actually know which side of that line you're on?

How Much I've Spent on Business Coaching as a PT (Was It Worth It?)

Since starting my cash based physical therapy practice in 2019, I have invested somewhere between sixty and eighty thousand dollars in business coaching across mini courses, group programs, one-on-one coaching, and a high-level mastermind.
In this episode I am giving you the full honest breakdown of what I spent, what I got out of each investment, the one program I somewhat regret, and the one I love so much I am reinvesting for another year.
This is not a episode about flexing on spending. This is a real accounting of what it looks like to invest seriously in your own growth as a physical therapy practice owner and business coach, and what I would tell my younger self if I could go back and do it differently.
We cover the full breakdown of what I have spent across almost seven years of investing in business coaching, the biggest takeaway about time versus money that changed how I think about every investment decision, what to look for in a coaching program and what red flags to run from, the green flags that tell you a program is worth the investment, and exactly when in your practice journey you are ready to invest in coaching and when you are not.
The most important thing I want you to take away from this episode is that time is a finite resource and money is not. Every month you spend trying to figure this out alone has a real cost — financial, emotional, and in terms of momentum. The question is not whether you can afford to invest in coaching. Physical Therapist Exit Strategy: How to Go From Employed to Full Time Practice Owner

One of the most common questions I get from clinicians building their own practice is some version of the same thing: "How do I know when it's time to leave my job?"

How to Get Physical Therapy Clients Without Referrals

I sat down with a practice owner this morning and she asked me the question I get more than almost any other: "Where do I even get clients from if it's not referrals and networking groups?"
Here's the honest answer: making referrals your main strategy is the most fragile way to build a cash based physical therapy practice. You don't control the input so you can't control the output. Your revenue is in someone else's hands on someone else's timeline. And that's the exact mechanism behind feast and famine.

Geriatric Physical Therapy: Building a Cash Based Practice | Dr. Sameep Contractor

Your Cash Based Practice Isn’t Growing (And It’s Not a Marketing Problem)

Why Clients Don't Book After Consults in Your Cash-Based PT Practice

You had a great consult call, they seemed interested, said it sounded like exactly what they needed, and then they ghosted. Or said they couldn't afford it. Or asked if you take insurance at the last minute. Sound familiar?
The problem is almost never your price. It's what happened at the end of that call. Most cash based physical therapy practice owners are running evaluations instead of consultations, and there's a huge difference between the two.
If you're hearing "I'll think about it" or "let me know" at the end of your discovery calls, this episode breaks down exactly why your consults aren't converting and what to change to start booking more evaluations consistently.
The biggest mistake? You're solving their problem on the call instead of leading them to a decision. When you give away the value of a full evaluation for free, there's no clear reason for them to book with you.
