What Gets Measured Isn't Always What Matters Most - Ep 192
2026/10/13
Are clinical metrics measuring what actually matters? In this episode, JACCP Editor-in-Chief Stuart Haines and new Associate Editor Ty Kiser sit down with Andrea Sikora and Brian Murray (University of Colorado) to discuss their editorial "Keeping the Good in Goodhart's Law: Bridging Key Performance Indicators to Comprehensive Medication Management," written in response to the Clinical Administration PRN Opinion Paper that proposes 15 key performance indicators (KPIs) for clinical pharmacy practice. Drawing on Goodhart's Law and McNamara's Fallacy, Sikora and Murray warn that easy-to-measure tasks like IV-to-PO conversion can crowd out the harder-to-quantify cognitive work of comprehensive medication management (CMM). Using vivid stories from the ICU, they make the case for pairing pharmacist-facing KPIs with structural accountability measures that protect time for the work that matters most.

Comparative Analysis of Nafcillin and Oxacillin: Implications for MSSA Treatment - Ep 186
2026/07/09
Host Dr. Michael Rybak interviews Drs. Alejandro Perez and Autumn Brenner (UNC Medical Center) about their retrospective study, “Incidence of Acute Kidney Injury with the Use of Nafcillin versus Oxacillin,” comparing 171 nafcillin patients with 467 oxacillin patients. The study found a higher AKI rate with nafcillin (33.3%) versus oxacillin (19.5%), along with higher hypokalemia, suggesting these agents may not be clinically interchangeable from a safety standpoint. They discuss confounding concerns around observed mortality and length-of-stay differences, offer mechanistic explanations, and recommend close monitoring of creatinine and potassium—especially in high-risk patients.
Pharmacotherapy and AI: Beyond the Fear, Towards Collaboration - Ep 185
2026/06/30
Host Dr. Tom Lodise interviews Dr. Marc Scheetz about his Pharmacotherapy editorial, “AI in ID Pharmacy: We Should Not Be Afraid to Put the CAR (Not the Cart) Before the Horse,” discussing how AI is already mainstream and increasingly used in healthcare. Scheetz uses the Industrial Revolution and the John Henry story to argue clinicians should not compete with machines but learn to make them work with us, as AI can outperform humans in many tasks. His optimistic view is that AI will reduce computer-bound work and preserve human connection in patient care. High-Value Interventions Primary Care Practices Might Be Overlooking: Clinical Pharmacy Services – Ep 180
2026/04/07
This podcast episode features an interview with Russell Phillips, Amie Pollack, and Dru Ricci from the Center for Primary Care at Harvard Medical School and authors of the Primary Care Investment Guide, with special guest, Todd Sorensen from the University of Minnesota College of Pharmacy. The Primary Care Investment Guide advocates for the integration of clinical pharmacists into primary care teams. The authors explain that their research used a mixed-methods approach, interviewing healthcare leaders from five states and conducting literature reviews, to identify sustainable strategies in primary care settings that enhance patient outcomes and reduce costs. Their findings highlight that clinical pharmacists provide a significant return on investment by managing chronic diseases, optimizing medications, improving medication access, and decreasing hospitalizations. The discussion emphasizes that successful integration can be facilitated by payment reform, moving away from fee-for-service models toward capitated payments and other alternative payment models that support optimal care delivery by primary care teams. Ultimately, the Guide provides a strategic framework for healthcare leaders and policymakers to justify and implement the provision of high-value services in primary care settings.    Funding for the Primary Care Investment Guide was provided by The Commonwealth Fund and the California Health Care Foundation.  You can download the Primary Care Investment Guide here:  https://thepcc.org/reports/2025-primary-care-investment-guide/
