Developing Safety Culture
2026/08/03
Safety culture is more than a poster on the wall or a box to tick. It is the foundation that makes a Safety Management System (SMS) work in practice. Without it, even the most detailed SMS is just a document sitting on a shelf.
In this episode, host Grenville Hudson turns the microphone on co-host Kathryn Harvey, who brings a background in safety management, safety culture, and human factors. Kathryn unpacks what safety culture really means, how organisations can assess where they sit on the culture maturity spectrum, and what leaders need to do to move the needle. She draws on research from Cox and Flynn (1998) and the safety culture maturity model developed by Professor Patrick Hudson to frame the discussion.
The episode is practical and direct. Kathryn covers values, trust, recruitment, investigations, and feedback loops and gives a realistic timeframe for how long genuine culture change actually takes.

Exploring AI in SMS
2026/07/23
Artificial intelligence (AI) is moving fast, and aviation safety is no exception. This episode explores where AI can add real value to safety management and where it still needs a human in the loop.
Alex Papley, an AI expert with 25 years of IT experience and founder of Hypergen, joins hosts Grenville Hudson and Kathryn Harvey to break down what AI actually is and what it is not. Alex explains the difference between traditional AI and generative AI (GenAI), why GenAI has opened the door for smaller operators, and why treating AI as a summarisation tool rather than a thinking tool is the key to getting good results.
The conversation covers practical use cases across the four pillars of safety: safety policy, risk management, safety assurance, and safety promotion and tackles the big questions around job security, data myths, and the ethics of handing over tacit knowledge to a machine.

Criminalisation of Aviation Incidents
2026/07/02
In this episode, hosts Grenville Hudson and Kathryn Harvey sit down with aviation lawyer Tony Power of Just Culture Consulting to explore one of the most pressing issues in modern aviation safety: the growing trend of criminalising honest mistakes.
Tony explains what Just Culture actually means and how it differs from a "no accountability" approach. He argues that blaming the person at the sharp end, such as the pilot, the controller, or the engineer, usually misses the real story. The episode shows how Australia's regulators, the Civil Aviation Safety Authority (CASA) and the Australian Transport Safety Bureau (ATSB), try to protect people who speak up, and where gaps in that protection still exist.
The back half of the episode digs into real cases, including a recent Queensland prosecution that ended in a jail sentence. It asks a hard question: if pilots fear prosecution for an honest mistake, will they ever feel safe enough to admit one?
