From Physical Therapist to CEO Part 1: Making the Shift Out of Patient Care
2026/10/02
For many physical therapy clinic owners, the goal is to eventually spend less time treating patients and more time building a business that can operate without them on the front lines. But getting out of the treatment room isn’t as simple as hiring another clinician and clearing your schedule. In this episode, we kick off a new series on the transition from clinician to CEO and break down what needs to happen before an owner can step away from patient care. We discuss why knowing your numbers is critical, how to think about the value of your time, and why reducing treatment hours doesn’t necessarily mean working less. In the early stages of scaling, those clinical hours often get replaced by work on client acquisition, sales, hiring, operations, and the systems needed to grow the business. We also talk about one of the biggest hurdles owners face when hiring: accepting that another clinician doesn’t have to practice exactly like you. The goal is to build a team capable of delivering great care while creating enough capacity for you to focus on growing the company. Ultimately, getting out of the treatment room requires knowing what you’re moving toward. If your goal is to build a larger, scalable business, you need to define the destination and work backward from there. Key takeaways: Know your numbers before trying to scale.Understand the true value of your time as an owner.Build strong systems for lead generation, sales, fulfillment, and financial tracking.Hire clinicians to create capacity for higher-level business work.Your employees don’t need to treat exactly like you to provide excellent care.Reducing clinical hours may initially mean replacing them with growth-focused work.Don’t sacrifice long-term growth just to maximize short-term profit from every employee.Decide what you ultimately want the business to become and build backward from that goal.Moving from clinician to CEO requires developing skills beyond patient care.Getting out of the treatment room isn’t about doing less. It’s about shifting your time toward the work that allows the business to grow beyond you. Marketing Part 3: How to Build a Scalable Patient Acquisition System for Your PT Clinic
2026/09/25
Getting patients through the door is one challenge. Building a patient-acquisition system that keeps working as your clinic grows is another. In part three of our patient acquisition series, we break down how to scale your marketing beyond the owner doing everything personally. A major focus of this episode is corporate contracts. We discuss how partnerships with employers, athletic organizations, and local businesses can create consistent exposure to groups of potential patients while giving clinics a growth channel that can be repeated, measured, and eventually delegated. We also compare corporate partnerships with more traditional physician referral relationships and explain why your ideal patient should influence which opportunities you pursue. From there, we move into the marketing numbers every clinic owner should understand. Generating leads is only the first step. You also need to know how many leads book, how many show up, how many become ongoing patients, what it costs to acquire them, and how much each patient is worth over time. One of the biggest takeaways: more leads are not always the answer. Improving follow-up, booking rates, show rates, and conversion can create meaningful growth without increasing your advertising budget. Key takeaways: How to make patient acquisition less dependent on the clinic ownerWhy corporate contracts can be a high-leverage growth channelHow corporate partnerships differ from physician referralsWhy your ideal patient should shape your marketing strategyThe key metrics behind leads, bookings, and conversionsWhy customer acquisition cost and patient lifetime value matterHow to decide which marketing channels deserve more investmentWhy lead nurture may be more important than simply generating more leadsHow small improvements in conversion can increase revenueWhy scalable marketing needs repeatable systems and SOPsThe goal is not just to find more patients. It is to build a marketing system that consistently attracts the right patients, converts them effectively, and can continue working as your team and clinic grow. Marketing Part 2: How to Turn Physical Therapy Leads Into Booked Evaluations
2026/09/11
Getting more leads does not automatically mean getting more patients. In part two of our patient acquisition series, we break down what happens after someone raises their hand and shows interest in your clinic. Once the lead comes in, the next challenge is responding quickly, qualifying the opportunity, overcoming objections, and moving the right person toward an evaluation or discovery visit. The fortune is in the follow-up. We discuss why speed-to-lead matters, why clinic owners often struggle when they delegate lead nurture too early, and how a simple, repeatable process can outperform an overly complicated sales script. We also walk through a practical example of an initial lead call, including the core questions that help determine whether someone is local, whether they need your services, and whether they are ready for the next step. Another major topic is the insurance question. The approach can look very different depending on whether you operate an insurance-based clinic or a cash-pay practice. Rather than immediately turning every conversation into a discussion about price or coverage, the goal is to understand the patient’s problem, establish trust, and determine whether your clinic can genuinely help. Finally, we talk about the psychology of selling physical therapy without feeling “salesy.” Effective sales is not about pressuring someone into treatment. It is about being confident in the value you provide, asking the right questions, clearly communicating that value, and giving the patient the opportunity to decide whether your solution is right for them. Key takeaways Why lead nurture is one of the biggest bottlenecks in patient acquisitionWhy responding to new leads quickly mattersHow to structure a simple initial qualification callThe questions to ask before booking an evaluation or discovery visitHow insurance-based and cash-pay practices may need different approachesHow to handle insurance questions without losing control of the conversationWhy trust and confidence play a major role in conversionThe importance of consistently doing the “boring work” that drives growthResources mentioned: Book a free strategy call: https://link.drbrianwolfe.com/widget/bookings/consultation-call-bwconsultingJoin the PhysioGrowth Mentorship Group: https://www.skool.com/physiogrowth-mentorship-group/aboutMore from Dr. Brian Wolfe & Dr. Owen Campbell: Blog: https://www.drbrianwolfe.com/blogPhysio Growth: https://www.physio-growth.comFront Row Back Row Podcast: Spotify: https://open.spotify.com/show/108ZxAjtdBE5HflGD6FUR1?si=MBsSU1x0Rw-BznTXfxIjxAApple Podcasts: https://podcasts.apple.com/us/podcast/front-row-back-row-podcast-presented-by-physiogrowth/id1723760895YouTube: https://www.youtube.com/@physiogrowth
Marketing Part 1: How to Attract More Patients to Your PT Practice
2026/08/14
Most physical therapy practices don’t have a lack-of-marketing problem. They have a lack-of-clarity problem. If your target patient is simply “anyone with a pulse and a wallet,” it becomes difficult to build a focused, repeatable client acquisition system. In Part 1 of our Client Acquisition series, we break down how to attract the right patients by getting specific about who you want to serve, understanding where those people spend their time, and creating messaging that speaks directly to the problems they want solved. Brian and Owen walk through the PhysioGrowth Perfect Patient framework and explain why two clinics offering similar services may need completely different marketing strategies based on geography, demographics, lifestyle, business model, and the type of patient they want to attract. They also discuss the differences between cash-pay and insurance-based practices, why trust matters at the provider, company, and service level, and how your stage of business should influence which acquisition channels you pursue. Key takeaways: Why “everyone” is not a useful target marketHow to define your perfect patientWhy your best current patients can reveal where to marketHow geography, demographics, and lifestyle shape your strategyWhy your messaging should focus on solving a specific problemThe three areas of trust patients evaluate before choosing a providerHow cash-pay and insurance-based clinics differ in conversionHow to think about physician referrals, community events, corporate relationships, sponsorships, and paid advertisingWhy lower-cost relationship strategies matter when you’re starting outHow better lead generation gives you more control over who you serve and how you growThis episode is Part 1 of our client acquisition series: Attract: Generate consistent leads from the right prospective patients. Convert: Turn those leads into scheduled evaluations. Scale: Build a predictable acquisition system that supports continued growth. Hiring Part 5: Why Great PTs Leave—and How Strong Leaders Retain Them
2026/08/07
Hiring a great physical therapist is only the beginning. The bigger challenge is building a clinic where strong employees want to stay, grow, and contribute. In the final episode of our hiring series, we discuss how leadership, communication, career development, and accountability shape employee retention. We also explain why clinic owners cannot rely on compensation alone to compete for great physical therapists. Brian and Owen break down how to create meaningful growth opportunities for employees who want to lead, as well as clinicians who want to develop their expertise without moving into management. They also discuss the importance of addressing employees who negatively affect the team, the patient experience, and the clinic’s performance. Key takeaways: Retention problems are often connected to leadership and culture.Clear communication, transparency, and recognition help employees stay engaged.Career paths should support both future leaders and clinically focused therapists.Strong employees want autonomy, accountability, and opportunities to contribute.Poor performers can damage team morale, patient retention, and revenue.Visits per evaluation and early patient drop-offs can help identify performance concerns.Smaller practices can compete with hospitals and large chains by creating a more purpose-driven workplace.Building a team that stays requires strong leadership, honest feedback, meaningful growth opportunities, and a commitment to protecting the clinic’s culture. Hiring Part 4: How to Onboard Physical Therapists and Build High Performers
2026/07/31
Hiring the right person is only the beginning. What happens during the first few days, weeks, and months can determine whether a new employee becomes a long-term high performer or starts looking for another opportunity. In part four of the PhysioGrowth hiring series, Brian Wolfe and Owen Campbell break down the systems they have used to onboard physical therapists across multi-location organizations, private practices, and growing clinical teams. They explain why structure matters more than information overload, how owners and clinical directors can create a strong first-day experience, and why new hires need to understand the purpose behind your policies, expectations, and culture. The conversation also explores the value of a structured two-week onboarding plan, 30-, 60-, and 90-day reviews, recurring check-ins, mentorship, appreciation, and clearly defined career paths. Whether you are hiring your first clinician or managing several locations, this episode provides a practical framework for helping new employees feel supported, understand what success looks like, and grow into valuable members of the organization. Key takeaways Why the owner or clinic leader should be present during the first days of onboardingHow a structured two-week onboarding plan reduces confusion and builds confidenceWhy new hires should learn your systems, EMR, expectations, values, and career opportunitiesHow to create a first-day experience that reassures employees they made the right decisionWhy every policy and process should be connected to a clear purposeHow to address cultural misalignment before it becomes a larger problemWhy performance conversations should feel structured rather than unexpectedHow recognition and appreciation can improve engagement and retentionWhy mentorship should be adjusted to the needs of each employeeHow career-path conversations help employees avoid feeling stuckWhy most onboarding and retention problems are often leadership problemsHow strong people systems reduce the need to constantly recruit and replace staffResources mentioned: Book a free strategy call: https://link.drbrianwolfe.com/widget/bookings/consultation-call-bwconsultingJoin the PhysioGrowth Mentorship Group: https://www.skool.com/physiogrowth-mentorship-group/aboutMore from Dr. Brian Wolfe & Dr. Owen Campbell: Blog: https://www.drbrianwolfe.com/blogPhysio Growth: https://www.physio-growth.comFront Row Back Row Podcast: Spotify: https://open.spotify.com/show/108ZxAjtdBE5HflGD6FUR1?si=MBsSU1x0Rw-BznTXfxIjxAApple Podcasts: https://podcasts.apple.com/us/podcast/front-row-back-row-podcast-presented-by-physiogrowth/id1723760895YouTube: https://www.youtube.com/@physiogrowth
