Why My Saturday Clinic Failed | Medical Practice Growth
2026/10/19
Opening a Saturday clinic sounded like a great way to improve patient access and grow my medical practice. Instead, weekend and after-hours appointments brought overtime costs, staffing challenges, patient pushback over a $25 fee, and follow-up responsibilities that extended beyond clinic hours. In this video, I share why I stopped our Saturday and after-hours clinics, what I underestimated, and what needs to change before we bring them back.I also discuss the hidden complexity of adding services, hiring clinicians, and opening new locations—from backup coverage and staff burnout to training, documentation, and managing urgent results. If you’re starting or scaling a private practice, this is a firsthand look at why every expansion deserves careful planning—and why focusing on one thing at a time can matter.Subscribe to InvestingDoc for real-world lessons on medical practice ownership and physician entrepreneurship.Learn more: https://investingdoc.comMy website 🖥️: https://www.investingdoc.comMy podcast Spotify Link🎧: https://open.spotify.com/show/3JxluO7mJOSE312G1Ul9dgApple Podcast Link 🎧: https://podcasts.apple.com/us/podcast/investingdoc/id1790276462My Instagram: https://www.instagram.com/investingdoc/My Twitter: https://x.com/InvestingDocMy Facebook Group: https://www.facebook.com/groups/investingdocReddit Group: https://www.reddit.com/r/PrivatePracticeDocs/Want to consult with me for your practice: Book here. https://investingdoc.com/consulting

Key Man Risk: Why Your Patients Only Wanting YOU Is a Problem

Consulting is paused while I get the MSO off the ground — back to it in a month or two.I'm Brad. I own a multi-million dollar private primary care practice and I help physicians start and scale their own. Subscribe if you want the business side of medicine nobody taught you in residency.Nothing here is medical, legal, financial, or tax advice. I'm a physician and a business owner, not your advisor. Every practice and every state is different — talk to your own counsel and CPA before making decisions.--------------A physician who's also my patient no-call-no-showed six times, showed up late to the appointment they finally kept, spent the visit venting about portal messages and work-life balance — and then told me that when they refer friends to me and I'm full, those friends should just go somewhere else. Not to my clinic. Somewhere else.None of that makes them a bad person or a bad doctor. They were in patient mode, which is exactly where they should have been. But as a business owner, that visit was one long message about what's broken in how I built this thing. That's hero syndrome. Key man risk. Star syndrome. Whatever you want to call it — when the patients only want you, you've built a bottleneck and called it a brand. In this one I break down the two paths you can pick at the start, why I picked the harder one, the worst possible middle ground to get stuck in (concierge-level care on fee-for-service pay), and why my practice is adding cash-pay and premium services in 2027.Chapters00:00 Hero syndrome, key man risk, star syndrome00:50 The physician-patient who inspired this01:16 Six same-day cancellations and professional courtesy02:30 "If they can't see you, go somewhere else"03:12 Two wolves: the two paths you get to choose04:29 Path two — boundaries from day one05:01 The tablet analogy: why changing the rules makes people furious05:20 You are the brand, and it doesn't scale06:17 The worst place to be: concierge care, fee-for-service pay06:43 Training patients and selling your PA07:30 Is a DPC / concierge hybrid the future?09:24 Listening to what patients are actually asking for11:03 She went online for HRT. 7 Business Lessons From a Small-Town Plumber That Made My Practice Millions
2026/08/24
My grandfather was a master plumber and master electrician in a Texas town of 2,400 people. He grew a plumbing, HVAC, and electrical company to 75+ employees — and almost everything I know about running and scaling a medical practice, I learned watching him do it.In this video I break down 7 lessons that transfer directly from a "boring" trades business to primary care, private practice, and DPC. Doctor Marketing 101: Get Referrals, Build Trust, and Grow Your Practice
2026/07/13
Most doctors are terrible at marketing, not because they're dumb, but because med school never taught them persuasion, trust building, or patient psychology. In this video, I break down exactly how I grew my primary care practice through grassroots marketing, referral relationships, and smart paid ads (after spending $1M+ on marketing). Learn how to win referrals as a specialist, when to run Google ads, why missed phone calls are your most expensive mistake, and how to market without feeling sleazy. Stop Running Your Medical Practice on Autopilot (It's Costing You Millions)
2026/07/06
Is your medical practice drifting instead of growing? In this video, I break down the biggest mistake I made in my own multi-million dollar primary care practice: running the business without intention. "I just want to be busier" is not a strategy. I'll show you how to set 30/60/90-day objectives, build standard operating procedures, escape survival mode, and create forward-looking plans like a publicly traded company — so your practice grows on purpose, not by accident.Whether you're in your first year fighting payroll, billing, and no-shows, or you're ready to scale from one location to ten, this video gives you the framework to define your goals, prioritize your dollars, and actually execute. My website 🖥️: https://www.investingdoc.comMy podcast Spotify Link🎧: https://open.spotify.com/show/3JxluO7mJOSE312G1Ul9dgApple Podcast Link 🎧: https://podcasts.apple.com/us/podcast/investingdoc/id1790276462My Instagram: https://www.instagram.com/investingdoc/My Twitter: https://x.com/InvestingDocMy Facebook Group: https://www.facebook.com/groups/investingdocReddit Group: https://www.reddit.com/r/PrivatePracticeDocs/Want to consult with me for your practice: Book here. https://investingdoc.com/consulting
Private Equity's "Second Bite" Scam Exposed: A $10M Cautionary Tale
2026/06/29
A physician reached out to share a story that every practice owner needs to hear. He co owned a specialty group, scaled it to multiple locations, and sold it for a headline price of $10 million. It sounded like the dream exit. Then reality set in.
In this episode I walk through exactly how that $10 million purchase price eroded down to just $2.5 million in his pocket. We break down the numbers step by step: the 40 percent equity rollover, the salary cut after becoming an employee in his own clinic, the performance metrics and the million dollar clawback, the legal and broker fees, and the taxes. Then came the worst part. The private equity firm loaded the new company with debt, missed the expansion goals, and filed bankruptcy, wiping out the rolled equity overnight.
This is a real story with the specialty, name, and state changed for privacy and shared for entertainment purposes. It is not always a disaster, and a second bite can pay off, but you have to understand the gotchas before you ever sign. If you are a physician thinking about an exit, a divestiture, or your retirement strategy, watch this before you take the meeting.
I am Brad. I own a private practice, I am a practicing physician, and I help people start and scale medical practices. Reach out if you want to talk through your own situation.

When To Hire an Office Manager for Your Medical Practice
2026/06/22
Still doing $25 an hour tasks while your time is worth $200? In this episode I break down exactly when it is time to bring on administrative help for your medical practice, and how to do it right.
I walk through the three management roles every growing practice should understand: the office manager who runs front desk and daily operations, the practice manager who handles HR, vendor contracts, compliance, and KPIs, and the administrator focused on high level decisions like opening new locations and hiring physicians.
I also share the revenue benchmarks I use (why you probably do not need a six figure practice manager until you cross roughly a million and a half in annual revenue), the hiring mistake I made by pulling someone from a stagnant clinic instead of a high growth background, and the systems that actually work: warm handoffs, defined roles, trial periods, one on one meetings, and KPIs.
If your schedule is full and you are waking up at four in the morning trying to fit everything in, you are the bottleneck. Here is how to get out of your own way.
I own a multimillion dollar primary care practice and help physicians start and scale their own. Drop your questions below.
