Does Estrogen Cause Breast Cancer? Dr. Avrum Bluming Challenges the Research
2026/10/19
Does estrogen cause breast cancer? And what does the research really say about hormone replacement therapy after breast cancer?
Hello, gorgeous! Today on The Midlife Realignment, I'm sitting down with one of my original teachers and mentors, Dr. Avrum Bluming, a medical oncologist and coauthor of Estrogen Matters, to challenge some of the most deeply held beliefs about estrogen, menopause, and breast cancer risk.
Dr. Bluming has spent decades studying hormone replacement therapy (HRT), including its use in women with a history of breast cancer. And let me tell you, this is a conversation every midlife woman deserves to hear.
We're digging into the controversial 2002 Women's Health Initiative findings that changed menopause treatment almost overnight, why Dr. Bluming believes the data have been widely misunderstood, and what women need to know when weighing the potential benefits and risks of estrogen therapy.
We also tackle one of the biggest questions in women's health: Can breast cancer survivors ever consider hormone replacement therapy?
Dr. Bluming walks us through research on HRT after breast cancer, including the HABITS study, and explains why he believes patients deserve individualized conversations instead of blanket dismissals. His interpretation remains controversial, and systemic HRT is generally not recommended for breast cancer survivors under current clinical guidance.
In this episode, we discuss:
The relationship between estrogen and breast cancer riskWhat the Women's Health Initiative actually studiedHormone replacement therapy after breast cancerWhy breast cancer recurrence and breast cancer mortality are different outcomesEstrogen's role in menopause symptom relief and bone healthBioidentical estrogen, estradiol, and PremarinThe lack of menopause education in medical trainingHow to advocate for yourself and ask better questions of your doctorIf you've ever felt dismissed when asking about menopause hormones, or you've wondered whether the fear surrounding estrogen tells the whole story, this episode is for you.
My goal is not to tell you what treatment to choose. It's to give you the information and confidence to become an active partner in your own healthcare.

Menopause insomnia had me waking up six times a night to pee, wide awake at 4 AM, and feeling like my brain had completely abandoned ship. And the crazy part? I'm a menopause expert who's spent nearly 30 years helping women navigate this exact hormonal f*****y.
Hello, gorgeous! In this episode of The Midlife Re-Alignment, I'm pulling back the curtain on my own two-month battle with insomnia, hormone fluctuations, brain fog, and a nervous system that seemed determined to keep me awake.
It all started when I switched my estrogen delivery method, ran into the estrogen patch shortage, and experimented with a different testosterone formulation. Suddenly, I was dealing with sleepless nights, urinary urgency, irritability, and exhaustion.
But as I started investigating, I uncovered another possible piece of the puzzle: the connection between estrogen, histamine, cortisol, and sleep.
I'm sharing what happened, what I tried, what didn't work, and what finally helped me get back to sleeping through the night.
In this episode, you'll learn:
Why menopause insomnia can become so disruptive when your hormones fluctuate. How estrogen changes may contribute to nighttime waking and urinary symptoms. What histamine and mast cells have to do with sleep, inflammation, and bladder sensitivity. Why cortisol may be part of the picture when you're wide awake at 4 AM.What happened when I tried a low-histamine diet for two weeks. How switching estrogen patch brands and testosterone formulations affected my symptoms. Why tracking symptoms and partnering with a knowledgeable practitioner matters more than ever. Most importantly, I want you to know that even when your hormones are throwing a full-blown shitstorm, you have options.
Your body is giving you information. You deserve to have someone listen.

Menopause and Heart Health: 7 Tests Every Midlife Woman Should Know About
2026/10/05
Heart disease is the number one killer of women—and yet most of us aren't thinking seriously about menopause and heart health until much later in life.
I want to change that.
In this episode of The Midlife Realignment, I’m breaking down what happens to your cardiovascular system as estrogen changes during perimenopause and menopause—and why midlife can be such an important time to start paying attention to your heart.
I walk you through seven metabolic and cardiovascular markers I want women to know about, including triglycerides, fasting insulin, fasting glucose, hemoglobin A1C, lipoprotein(a), hs-CRP, and homocysteine. I also explain the coronary artery calcium (CAC) score and how it can give you a completely different kind of information about your cardiovascular health.
But testing is only part of the conversation.
We’re also getting into insulin resistance, visceral fat, protein, fiber, strength training, walking, continuous glucose monitors, cholesterol, and the evolving conversation around hormone replacement therapy and cardiovascular health.
My goal isn't to scare you. It's to help you understand your body well enough to walk into your doctor's office informed, prepared, and ready to advocate for yourself.
Because protecting your heart in midlife doesn't start at 65. It starts now.
In this episode, you'll learn:
• Why menopause and heart health are so closely connected
• How changing estrogen levels may affect cardiovascular risk
• Seven cardiovascular and metabolic markers to discuss with your provider
• What a coronary artery calcium score can—and can't—tell you
• Why fasting insulin matters even when fasting glucose looks normal
• How insulin resistance and visceral fat connect to cardiovascular health
• Why protein, fiber, muscle, and walking matter in midlife
• How a continuous glucose monitor can help you understand your metabolic response to food
• What to discuss with a hormone-literate provider about HRT and heart health
Your heart deserves your attention long before there's a problem. This episode will help you start asking better questions.

Breast Implant Illness or Menopause? The Symptoms Natalie Jill Almost Missed
2026/09/28
What if the symptoms you’ve blamed on menopause aren’t just menopause?
In this episode of The Midlife Realignment, I sit down with fat loss expert, sports nutritionist, and Midlife Conversations host Natalie Jill for an incredibly candid conversation about breast implant illness, breast implant removal, inflammation, toxic load, autoimmune conditions, gut health, oral health, and rebuilding your body in midlife.
Natalie had breast implants for more than three decades and went through five sets of implants, repeated capsular contracture, ruptures, and years of health challenges. When blood sugar dysregulation, inflammation, gut problems, autoimmune issues, and other seemingly unrelated symptoms began piling up, she initially thought perimenopause and menopause might be to blame.
Eventually, a ruptured implant forced her to take another look.
Natalie shares why she finally decided to remove her implants rather than replace them again, what she says changed in her body after explant surgery, and why the experience completely transformed the way she thinks about health.
But this conversation goes far beyond breast implants.
We talk about the concept of “toxic load,” why Natalie believes seemingly unrelated health problems can sometimes be connected, what she learned about her oral microbiome and an old root canal, and why becoming a detective about your own health can be especially important in midlife.
Natalie also opens up about the emotional side of breast implant removal after having implants since age 19, adjusting to her changing body, recovering from an L5-S1 disc replacement, and rebuilding strength in her 50s.
We also discuss:
• Breast implant illness and the symptoms Natalie experienced
• Breast implant rupture and capsular contracture
• Why she initially blamed her symptoms on menopause
• Her experience with blood sugar dysregulation and inflammation
• Breast implant removal and what happened afterward
• Autoimmune conditions, including celiac disease and Hashimoto’s
• Gut health, oral health, mold exposure, and “toxic load”
• Root canals and the oral microbiome
• Her L5-S1 disc replacement
• Why strong glutes and core strength matter as we age
• The functional exercises Natalie believes women should prioritize
• Why feeling better may need to come before pushing harder in the gym
If you’ve ever thought, “Something is wrong with my body, but I can’t figure out what,” this conversation will encourage you to keep asking questions and looking at the bigger picture.
Because sometimes what looks like “just menopause” deserves a closer look.

Life After Hysterectomy: I Had No Idea How Much Pain Was Costing Me

What does life after hysterectomy really look like—and what happens when removing the physical pain gives you the clarity to change everything else?
It’s been almost one year since my hysterectomy, and I’m taking you behind the scenes of what actually led me to surgery, what my hysterectomy recovery was like, and the changes I never could have predicted.
Before surgery, I had normalized an unbelievable amount of pain. I was spending multiple nights a week in bed with severe cramping, migraines, inflammation, exhaustion, and pain that traveled from my pelvis into my back. Imaging revealed multiple fibroids, and after surgery I learned I also had adenomyosis.
But this episode isn’t just about my hysterectomy.
Once my body wasn't fighting pain every day, I could finally see how burned out the rest of my life had become.
I rebuilt my business. I reduced my team. I walked away from commitments that no longer served me. I stopped outsourcing my decisions to other people. And I created enough space in my life for opportunities I had dreamed about—including my TEDx talk and women's health advocacy work in Washington, D.C.
I’m sharing what this year taught me about life after hysterectomy, burnout, menopause, self-advocacy, trusting your body, and recognizing the difference between a full-body “hell yes,” a “hell no,” and a “not right now.”
If you've been pushing through pain, exhaustion, or a life that no longer feels like yours, this episode is your reminder: you are allowed to change your mind, clear the noise, and build your next chapter differently.
Could Insulin Resistance Be Increasing Your Cancer Risk? What Women Need to Know

What if some of the biggest warning signs for cancer are showing up years before a diagnosis — and we're simply not looking for them?
In this episode, I sit down with Dr. Lorna Brudie, a dual board-certified OB-GYN and gynecologic oncologist who spent years treating women at the point when cancer was already a reality. Today, she's looking at women's health through a very different lens: prevention.
We dig into the connection between insulin resistance and cancer risk, chronic inflammation, visceral fat, metabolic dysfunction, hormones, and the warning signs that can develop long before conventional bloodwork raises a red flag.
Dr. Brudie also shares her own shocking experience discovering that her fasting insulin was 31 — despite weighing just 119 pounds, exercising regularly, strength training, and being hormonally optimized. It's an important reminder that you can look healthy on the outside while metabolic dysfunction is quietly developing underneath.
We also unpack the Women's Health Initiative and the fear it created around hormone replacement therapy, why Dr. Brudie believes hormones have been unfairly blamed for cancer, what women with a history of cancer need to know about HRT, and why cancer survivorship should mean much more than simply being cancer-free.
We cover:
• The connection between insulin resistance, inflammation, and cancer risk
• Why fasting insulin may reveal problems that glucose doesn't
• How metabolic dysfunction can exist at any body size
• The role visceral fat may play in hormone-driven cancers
• Why Dr. Brudie believes cancer prevention needs to start years earlier
• What the Women's Health Initiative got wrong about hormone therapy
• Bioidentical hormone replacement therapy and cancer risk
• Hormone therapy after cancer treatment
• Why testosterone deserves a bigger place in women's health
• GLP-1 medications, metabolic health, and emerging cancer research
• Why cancer survivors deserve comprehensive hormonal, metabolic, cardiovascular, sexual, and psychological care
• How to advocate for yourself when you're told your labs are “normal”
If you've ever been told everything looks normal even though you know something isn't right, this conversation will give you a completely different way to think about your long-term health.
Because cancer doesn't start with a diagnosis — and prevention shouldn't start there either.

How to Ask Your Doctor for HRT (and What to Say If They Tell You No)
2026/08/31
If you want hormone replacement therapy but don't know how to ask your doctor for HRT—or what to say when you're told your labs are "normal"—this episode is for you.
I hear from women every day who finally work up the courage to ask about HRT, only to be told they're too young, their labs look fine, they should "wait and see," or hormones are too risky.
So I'm giving you the actual words to use.
In this solo episode of The Midlife Realignment, I walk you through how to prepare for an HRT appointment, what to ask your doctor, and how to advocate for symptom-based treatment when bloodwork doesn't tell the whole story.
I'm also breaking down how to ask about estrogen, progesterone, vaginal estrogen, and testosterone; what to say when your doctor immediately says no; how to discuss concerns about breast cancer and the Women's Health Initiative; and when it's time to find a menopause specialist or hormone-literate provider.
Plus, I'll show you how to track your symptoms before starting HRT and what to bring to your 8–12 week follow-up so you and your provider can determine whether your dose or delivery method needs to change.
You don't need a medical degree to advocate for yourself. You need the right questions, the right language, and the confidence to use them.

What Perimenopause Hormones Are Really Doing to Your Brain with Dr. Sarah E. Hill
2026/08/24
What if the brain fog, anxiety, sleep problems, mood shifts—and even changes in how you see your partner—aren’t random?
In this episode of The Midlife Realignment, I sit down with research psychologist and women’s hormone expert Dr. Sarah E. Hill to unpack what perimenopause hormones are really doing to the brain.
Sarah explains how estrogen and progesterone influence far more than our reproductive systems. They affect how quickly our brains process information, how emotionally sensitive we feel, our sexual desire, our response to stress, and even the qualities we find attractive in a partner.
And when those hormones begin fluctuating and declining during perimenopause? You can feel like somebody changed the operating system without telling you.
We talk about why perimenopause brain fog can appear even when your periods are still regular, how declining progesterone may contribute to anxiety and sleep problems, and why declining estrogen can affect memory and cognitive sharpness.
We also get into hormone therapy—including the important distinction between progesterone and synthetic progestins—and Sarah shares her own decision to begin estrogen and progesterone in her 40s.
Plus, Sarah explains something every woman worried about her brain needs to hear: your brain is remarkably adaptable. Hormonal change does not automatically mean permanent cognitive decline.
We also explore fascinating emerging research into vagus nerve stimulation, stress resilience, exercise, sleep, nutrition, and other ways we may be able to support the brain through perimenopause and menopause.
If you've ever thought, "Why don't I feel like myself anymore?" this conversation may completely change the way you understand what's happening inside your brain.

Menopause and Marriage: What Your Husband Really Thinks About Your Changing Body

What does menopause actually look like from your husband's side of the bed?
In this episode of The Midlife Realignment, I'm doing something I've never done before: bringing my husband of almost 23 years, Jeremy Horn, onto the podcast to talk candidly about menopause and marriage from a man's perspective.
Jeremy has been beside me through pregnancy, postpartum, health challenges, perimenopause, menopause, sleepless nights, changing hormones, changing bodies, brain fog, and everything in between. And today, I'm asking him the questions women really want answered.
Does your husband notice your weight gain, cellulite, or changing body as much as you think he does? What happens to a marriage when libido disappears? What should husbands understand about brain fog, sleep problems, mood changes, and other menopause symptoms? And what does it actually mean to support your wife through menopause?
We also get into something I see constantly in my practice: women feeling like they need permission to invest in their own health. Jeremy shares why he believes refusing to help your partner get the care she needs can be incredibly shortsighted—and why supporting her ultimately benefits the entire relationship.
In this episode, we discuss:
How menopause can affect marriage and intimacyWhat husbands really notice about women's changing bodiesWhy sleep can become such a major issue during perimenopauseWhat men need to understand about menopause symptomsBrain fog, forgetfulness, libido, weight gain, and body imageWhy menopause can create unexpected relationship tensionHow to support your wife through menopauseWhy women shouldn't have to ask permission to prioritize their healthThe importance of communication, humor, friendship, and respect in a long-term marriageWhy getting the right menopause care can benefit both partners
Jeremy's biggest message to men is simple: the woman you love may be struggling with far more than she's telling you.
This episode isn't just for women. Send this one to your husband or partner.
Because menopause doesn't only happen to one person—it happens inside a relationship.
Progesterone Intolerance: Why Progesterone Side Effects Can Make You Feel Worse on HRT
2026/08/10

If progesterone is supposed to help you sleep, calm your nervous system, and balance your hormones… why can it make some women feel absolutely terrible?
In this episode of The Midlife Realignment, I’m breaking down progesterone intolerance—one of the most frustrating and misunderstood problems I see in women using hormone replacement therapy (HRT).
If you’ve experienced progesterone side effects like extreme grogginess, depression, anxiety, brain fog, bloating, constipation, diarrhea, or the feeling that you’ve been hit by a truck after taking oral progesterone, there may be a biological reason.
I explain how oral progesterone is metabolized into the neurosteroid allopregnanolone, how it interacts with GABA receptors in the brain, and why two women can take the exact same progesterone dose and have completely opposite reactions.
I also break down why 100 mg of progesterone may be far too much for one woman—and potentially not enough for another.
🔥 In this episode, you’ll learn:
What progesterone intolerance is and the symptoms to watch forWhy progesterone can cause anxiety, depression, grogginess, and GI symptomsHow allopregnanolone and GABA affect your response to progesteroneWhy PMDD, postpartum depression, ADHD, anxiety, and medication sensitivity may be important cluesWhy your progesterone dose may need to be lower—or, surprisingly, higherThe differences between oral, vaginal, transdermal, and local progesterone/progestogen optionsWhy the route of administration can completely change your experienceWhat the DUTCH test may reveal that a standard progesterone blood test doesn’tQuestions to bring to your provider if progesterone is making you feel worse
If you’ve been told to “just give it more time” even though you feel miserable on progesterone, this episode will give you the language and information you need to have a much more productive conversation with your provider.
You don’t necessarily have to give up on HRT because one progesterone protocol isn’t working. Your dose, formulation, timing, metabolism, and route of administration all matter.
It’s time to stop assuming there’s one standard hormone protocol for every woman—and start finding the one that actually works for YOUR body.

ADHD in women doesn't always look the way we've been taught—and for many women, it isn't discovered until perimenopause.
In this episode, I sit down with hormone expert and bestselling author Dr. Jolene Brighten to unpack why ADHD is so often missed in women, how hormonal shifts can suddenly make symptoms impossible to ignore, and why so many women are misdiagnosed with anxiety, depression, or simply told they're "doing too much."
We dive deep into the fascinating relationship between estrogen, progesterone, testosterone, dopamine, gut health, and brain function. Dr. Brighten also explains why some women experience severe progesterone sensitivity, why ADHD symptoms often worsen during perimenopause, and the lifestyle, nutrition, and supplement strategies that can help support focus, mood, and cognitive health.
Whether you've been diagnosed with ADHD, suspect you may have it, or have noticed dramatic cognitive changes during perimenopause, this conversation will help you better understand what's happening inside your body—and how to advocate for the care you deserve.
In this episode you'll learn:Why ADHD in women is frequently overlookedHow hormones influence ADHD symptomsWhy perimenopause can unmask lifelong ADHDThe connection between progesterone sensitivity and neurodivergenceHow gut health affects brain healthNutritional strategies for supporting ADHDSupplements that may support cognition and focusThe role testosterone plays in women's brain healthWhy individualized hormone therapy matters

• Why TSH alone isn't enough to evaluate thyroid health
• The thyroid labs every woman should understand
• Why Hashimoto's is often a gut issue before it's a thyroid issue
