Still Hungry On A GLP-1? Here Is What You Can Do!
2026/10/19
Hunger coming back on a GLP-1 can feel scary, especially if you’ve been told the medication should “shut off” your appetite forever. I walk through what’s actually happening in the body, starting with ghrelin, the main hunger hormone, and why hunger is a necessary survival signal that can get dysregulated by chronic calorie restriction, weight cycling, and other metabolic conditions. We also talk about why a little appetite returning can be normal and expected, and how to spot the difference between appropriate hunger and the kind that keeps you thinking about food all day. From there, I get practical. We start with the biggest levers that often get missed: eating earlier in the day instead of saving calories for dinner, building meals around enough protein to protect muscle and support satiety, and increasing fiber (goal: 25 to 35 grams per day) to improve fullness, blood sugar, heart health, and GLP-1 related constipation. I also cover healthy fats, hydration (because thirst can masquerade as hunger), and why consistent, balanced meals beat “powering through” the day and getting ambushed at night. If you’ve tightened up nutrition and you still have excessive hunger, we move into medical options to discuss with your clinician, including adjusting your GLP-1 dose and considering combination obesity medicine. Finally, we zoom out to common hidden drivers like poor sleep and obstructive sleep apnea, chronic stress and elevated cortisol, other medications that can affect weight or appetite, and exercise volume that may unintentionally increase hunger. If you want a clear checklist and a calmer framework for troubleshooting GLP-1 hunger, listen now, share with someone who needs it, and subscribe and leave a review so more people can find the show. Hungry Gut Obesity Phenotype
2026/10/15
We break down the “hungry gut” obesity phenotype and why some people feel hungry again soon after eating even when the meal is satisfying. We share the treatment options that best match fast gastric emptying, plus a simple protein-first habit that can extend fullness between meals.  • Why obesity treatment is not one size fits all  • What “hungry gut” means and how imaging studies identified faster gastric emptying  • How fast gastric emptying can drive earlier hunger and more snacking  • Why GLP-1 agonists fit hungry gut by slowing gastric emptying  • Common GLP-1 side effects linked to slower emptying and why that can be beneficial here  • Why gastric bypass is the preferred surgery option over sleeve gastrectomy for hungry gut  • When gastric bypass is often recommended for obesity with type 2 diabetes  • Lifestyle strategy of starting meals with protein up to an hour before eating  • Combining phenotypes and using a combination treatment plan with an obesity medicine physician  Subscribe and stay tuned so we can learn about the next two obesity phenotypes.  If you found this information helpful, please share with a friend, family member, or colleague.  Please subscribe and write a review.  More information about GLP-1s: General Info: https://youtu.be/J4LzMNEzyTs?si=xUj0GX77rOlcs5rS How to Start: https://youtu.be/QItUBNUau7M?si=NCsVVFiORlkjNJoI Long Term Safety: https://youtu.be/jMAzVmaLqic?si=TlT32F8OMDTSCS0b Nausea: https://youtu.be/07-QFm71YMo?si=ySEQtNdf1Pph9-L2 Constipation: https://youtu.be/rog26mYgg9g?si=ER3jharJZ8F6K4GJ Reflux: https://youtu.be/cwJfaq6CA-o 👩🏼‍⚕️ Live in Missouri? Binge Eating Disorder Is Treatable And You Are Not Alone
2026/10/12
If you’ve ever felt out of control around food and then drowned in guilt afterward, you’re not weak and you’re not alone. We’re talking about binge eating disorder (BED), the most common eating disorder and one of the most underdiagnosed, largely because it’s hard to say out loud and easy for clinicians to miss. I break down what actually defines BED: recurrent binge episodes within a short time window, a painful loss of control, secrecy and shame, and real distress, without compensatory behaviors like vomiting or over-exercising. We also dig into why it happens, including genetics, environment, restrictive dieting patterns, trauma, food insecurity, chronic stress, and the brain’s reward pathway that can make urges feel overwhelming. Along the way, I explain why you can’t diagnose BED by looking at someone, and why BED is a biologic condition rather than a “willpower problem.” You’ll also hear how diagnosis works in real life, including the key time frame and frequency criteria and the BED-7 screening questions that can guide a conversation with your clinician. Then we get practical about treatment for binge eating disorder: evidence-based therapy like cognitive behavioral therapy (CBT) and interpersonal therapy, FDA-approved medication options like lisdexamfetamine (Vyvanse), and other medications that may help when anxiety, depression, or ADHD overlap. I also share a careful perspective on GLP-1 medications and why caution matters for some people. If any of this resonates, take the next step: talk with your primary care clinician, obesity medicine physician, therapist, psychiatrist, eating disorder specialist, or dietitian. Subscribe, share this with someone you care about, and leave a review so more people can find real help. Hungry Brain Obesity Phenotype
2026/10/08
Ever watched two people start the same GLP-1 medication like Wegovy or Zepbound and get wildly different results, then wondered what you’re missing? We’ve seen it too: some patients lose an incredible amount of weight, while others barely budge or even gain. That isn’t just “willpower” or “motivation.” A big part of the answer is obesity phenotypes, and today we dig into phenotype number one: hungry brain. Hungry brain obesity is the pattern of persistent, excessive hunger even after a meal that should have been satisfying. When your brain keeps sending “eat more” signals despite adequate nutrition, it becomes incredibly hard to stick to portions, meal timing, and long-term lifestyle changes. We explain what that experience looks like in real life, why a one-size-fits-all plan can backfire, and how a phenotype-based approach can make weight loss treatment feel less like a daily fight. Then we get practical: we walk through targeted anti-obesity medications that may fit hungry brain, including phentermine and Qsymia (phentermine plus topiramate), along with key safety considerations, monitoring needs, and who should not use stimulant-based therapy. We also cover why sleeve gastrectomy can be an optimal bariatric surgery choice for hungry brain due to stomach stretch signaling and the hunger hormone ghrelin, and we share the lifestyle strategy that matches the same biology: high-volume, high-fiber eating with foods like fruits, vegetables, and whole grains. Subscribe for the next three releases on the remaining obesity phenotypes, share this with someone who’s stuck, and leave a review so more people can find trustworthy metabolic health guidance. 👩🏼‍⚕️ Live in Missouri? PMOS Is The New PCOS: Emphasis On Metabolic Health
2026/10/05
We break down why PCOS is now called PMOS and why the new name points to the real issue: metabolic health and insulin resistance, not just ovaries. We share how to spot the symptoms, how doctors diagnose it using the Rotterdam criteria, and how to build a treatment plan that fits your goals, including fertility. • why the name changed from PCOS to PMOS and what it clarifies • common symptoms like irregular cycles, hormonal acne, chin hair, scalp hair thinning, weight gain, skin changes, mood shifts • why birth control can mask cycle-related symptoms and what that means for diagnosis • what drives PMOS: genetics, environment, lifestyle factors, insulin resistance, androgen hormones • Rotterdam criteria explained: two out of three categories for diagnosis • when labs help and when clinical signs are enough • when transvaginal ultrasound is useful and when it is not needed • lifestyle foundations: sleep, protein and fiber, movement, hydration, mental health support • medication options: hormonal contraception, metformin, spironolactone, fertility meds like letrozole and clomiphene • why regular screening matters: prediabetes, type 2 diabetes, fatty liver disease, cholesterol, blood pressure, sleep apnea, mental health • long-term risks including endometrial cancer with untreated irregular cycles Please share with someone who may have PMOS or does and is not having adequate treatment. Obesity Phenotypes & Why Ozempic May Not Be Right For You
2026/10/01
We break down why Ozempic and other GLP-1 medications can lead to wildly different weight loss outcomes from person to person. We explain the four obesity phenotypes and how matching treatment to your biology can make lifestyle changes, medications, and even surgery far more effective.  • why “hyper responders” and “hypo responders” exist with semaglutide and similar medications  • what an obesity phenotype is and why many people are a blend  • hungry brain signs and why high-volume high-fiber meals help  • hungry brain options including phentermine, Qsymia, and sleeve gastrectomy  • hungry gut signs, gastric emptying studies, and strategies like protein starters  • why GLP-1 agonists and Roux-en-Y gastric bypass fit hungry gut physiology  • emotional hunger patterns, the role of CBT and therapy, and why surgery is not the solution  • Contrave as a medication option for reward-driven eating  • slow burn metabolism, resting metabolic rate, and lessons from intense weight loss  • resistance training, protein, and NEAT as the best slow burn tools  If you found this information helpful, please share with a friend, family member, or colleague. Supplements On GLP-1s
2026/09/28
ProCare Supplements: 10% off CODE: MetabolicHealth ProCare Iron + vit C supplement get 10% off with code: MetabolicHealth Fatigue on GLP-1 YouTube and Podcast If you’re taking a GLP-1 medication like Wegovy or Zepbound, the supplement ads can feel endless and urgent and honestly, confusing. I break through the noise with a simple goal: help you spend less money, take fewer unnecessary pills, and make safer choices that actually support your metabolic health while your appetite and portions may be changing. We start with the most important framework: supplements are not regulated like prescription medications. That means quality varies, labels can be misleading, and “natural” does not mean safe. I share exactly what I look for when I’m choosing a supplement brand, including third-party testing and seals like USP, NSF, and ConsumerLab. We also talk about why you should always tell your doctor or pharmacist what you’re taking, how higher doses can backfire, and how too many products can raise the risk of interactions or liver and kidney strain. Then I walk through a practical, patient-centered shortlist of supplements I commonly consider in obesity medicine: multivitamins when intake is smaller, prenatal vitamins when pregnancy is possible, vitamin D supplementation and who is at higher risk, iron deficiency symptoms and why ferritin matters, plus when B12 or magnesium might be worth checking. We also cover protein supplements to protect muscle during GLP-1 weight loss, creatine dosing paired with strength training, collagen for specific goals, and gut and hydration tools like fiber and electrolytes. We wrap with two popular insulin resistance options, berberine and inositol, including where they may fit and when to pause and talk with your clinician. If this helps you, subscribe, share it with someone getting lost in supplement marketing, and please leave a review so more people can find the show and build better metabolic health. 👩🏼‍⚕️ Live in Missouri? What To Eat With Type 2 Diabetes With Wendy Castle, RD
2026/09/24
If you or someone you love has diabetes and would like to learn how to optimize nutrition to help manage this chronic condition, then I highly recommend Wendy Castle and her team at One Bite Nutrition. Instagram: https://www.instagram.com/one_bite_nutrition/ Website: https://onebitenutrition.com/ Diabetes advice online can feel like a shouting match, and it often leaves people thinking they have to fear carbs, chase perfect numbers, or overhaul their entire life overnight. We sit down with Wendy Castle, a registered dietitian and certified diabetes educator, to bring the conversation back to what actually helps: clear education, realistic nutrition skills, and a plan you can stick with. We start by separating type 1 diabetes from type 2 diabetes so the “why” behind treatment finally makes sense. Then we get practical about food: how to read a nutrition label, why “total carbohydrates” matters more than just sugar, and how fiber, protein, and fats can help smooth out blood sugar spikes so your post-meal curve looks more like a hill than a mountain. Wendy also shares why there’s rarely a single best diabetes diet, and how personalized meal planning can fit allergies, culture, and real-life schedules. Next, we tackle blood sugar monitoring and the mental load that can come with it. You’ll hear a patient-centered way to decide whether checking helps you make positive changes or whether it derails you, plus a staggered testing method that provides better insight without constant finger pokes. We also cover non-food factors that impact glucose, including hydration, movement, stress, illness, and medication pitfalls, along with simple actions like water and walking that can help bring high readings down. If you want smarter type 2 diabetes management strategies, insulin resistance education, and lifestyle tools that support better A1C without shame, hit play. Subscribe, share with someone who needs calmer diabetes guidance, and leave a review so more people can find trustworthy metabolic health information. 👩🏼‍⚕️ Live in Missouri? Want to be my patient? Plenity, A Weight Loss Device
2026/09/21
A weight loss capsule that isn’t a drug sounds like a gimmick until you understand what Plenity actually is: an FDA-cleared medical device that you swallow, hydrate with water, and then pass through your system without absorbing it. We walk through how Plenity uses cellulose and citric acid to form a gel-like material that takes up space in your stomach so you feel full sooner and naturally stop eating earlier. If you’re looking for a practical tool for portion control, appetite support at meals, and a stepwise approach to metabolic health, this option deserves a closer look. We also get specific about who Plenity is for and why it fills a unique gap in obesity medicine. Plenity is cleared starting at BMI 25, which may matter if you’re in the BMI 25 to 26.9 range and feel stuck because you do not meet the usual prescribing thresholds for many anti-obesity medications. We talk realistic expectations for weight loss, including the typical average around 6% total body weight loss over about six months and why people who notice early benefits often have the strongest results. From side effects like bloating and feeling overly full to important cautions around pregnancy, GI tract concerns, prior GI surgery, and GERD, we lay out the safety considerations in plain language. We also cover a key practical point: Plenity may affect absorption of other medications, so timing matters. Finally, we discuss cost and access since insurance coverage is unlikely, plus lower-cost alternatives like psyllium fiber (Metamucil) or chia seeds that can mimic the same “space-filling” strategy. If this helped you think more clearly about Plenity, share the episode with someone who’s navigating weight loss and metabolic health, then subscribe and leave a review so more people can find trustworthy guidance. 👩🏼‍⚕️ Live in Missouri? What Is Metabolic Health? Dr. Francavilla Interviews Dr. Ogle
2026/09/07
We break down what metabolic health actually means and why it deserves more attention than the number on the scale. We connect the dots between common diagnoses and one underlying metabolic pattern, then lay out practical ways to test, treat, and improve it.  • choosing “metabolic health” to focus on health outcomes over aesthetics  • why BMI misses risk and cannot define metabolic health  • a working definition of metabolic health as nutrient processing and energy use  • metabolic syndrome criteria and what they reveal about risk  • related conditions like prediabetes, type 2 diabetes, MASLD fatty liver disease, PCOS, sleep apnea, hypertension, dyslipidemia  • key labs and screening tools including A1C, fasting glucose, lipid panel, liver enzymes, sleep apnea testing  • why improving metabolic health lowers cardiovascular disease risk and supports better quality of life  • treatment approach that blends protein, fiber, hydration, regular meals, post-meal movement, sleep, stress management, and medications like GLP-1s  • when CGMs help and when they become too much data  • the core takeaway that metabolic disease is not a personal failure and early action matters  If you found this information helpful, please share with a friend, a family member, or a colleague.
