EUS-GJ: When the Stomach Needs a Shortcut — with Professor John Leeds
2026/09/06
Endoscopic ultrasound-guided gastrojejunostomy is moving rapidly from specialist innovation towards an established therapeutic EUS technique. But who should actually be doing it, where should it be performed, and which patients should we be considering? I am a PB fellow and heard this very question being asked by a consultant - hence I thought I would find someone to address it.After all if a treatment is better, knowing when to refer for it with improved awareness may improve the lives of our patients.
In this episode of the Northern Gut Hub Podcast, Professor John Leeds joins us to explore the evolution of EUS-guided gastrojejunostomy, its current indications and where it may fit alongside surgery and enteral stenting.
We discuss patient selection, training and operator experience, service requirements, when to refer, and how EUS-GJ might develop within UK practice.
A practical discussion aimed at gastroenterology registrars, consultants and anyone interested in interventional endoscopy.
Professor Leeds & Pancreatobillary Medicine Made Practical Part 2 of 2
2026/06/07
In part two, Prof John Leeds discusses IgG4-related disease, pancreatic exocrine insufficiency, and the common traps in interpreting investigations without clinical context.
The episode covers why IgG4 disease is more than autoimmune pancreatitis, how it can mimic pancreatic cancer, when MDT input matters, and how to approach uncertain cases. We also discuss PEI, faecal elastase pitfalls, PERT dosing, fat-soluble vitamin deficiency, and why history should always guide the test you choose.
A practical pancreatobiliary episode for gastroenterology registrars, medical registrars, and anyone managing complex pancreatic or biliary presentations.
Professor Leeds & Pancreatobiliary Medicine Made Practical Part 1 of 2 : Assessing Jaundice and imaging and more
2026/05/31
In this episode of the Northern Gut Hub podcast is joined by Professor John Leeds, pancreatobiliary physician at the Freeman Hospital, for a practical discussion on the fundamentals of jaundice and suspected pancreatobiliary disease.
This first part focuses on how to approach a jaundiced patient safely and logically, starting with the basics: history, examination, blood tests, and choosing the right imaging. The discussion covers how to distinguish pre-hepatic, hepatic, and post-hepatic causes of jaundice, the importance of symptoms such as itch, dark urine, pale stools and weight loss, and why clotting matters before any intervention.
Professor Leeds also explains the role of ultrasound, CT, pancreatic protocol CT, and endoscopic ultrasound in assessing biliary obstruction, pancreatic cancer, small bile duct stones, cholangiocarcinoma and pancreatic masses.
A useful episode for gastroenterology registrars, medical registrars, and anyone wanting a clearer approach to pancreatobiliary referrals.
